Consumer Directed Services (CDS)

At PathFinders Family Coaching, we believe every individual has God-given potential. 

In addition to our private-pay coaching programs, Coach Joy is available to serve families who participate in Texas Medicaid Waiver programs through the Consumer Directed Services (CDS) option.

What is Consumer Directed Services (CDS)?

Some Texas Medicaid Waiver programs allow individuals and families to choose who provides certain approved services. Through the CDS option, families have greater flexibility in selecting qualified providers who best meet their loved one's needs.

If your loved one participates in a qualifying Texas Medicaid Waiver program—such as HCS, TxHmL, CLASS, STAR Kids, STAR+PLUS, or another eligible program—and has approved habilitation services available through the Consumer Directed Services (CDS) option, you may have the opportunity to choose Coach Joy to provide approved supports, consistent with your authorized service plan and program requirements.

🌿 Habilitation & Skill-Building Supports

Through the CDS pathway, families may choose Coach Joy to provide approved habilitation and skill-building supports that promote greater independence and participation in everyday life.

Supports may focus on areas such as:

• Daily living routines

• Communication and social interaction

• Self-help and adaptive skills

• Community participation

• Organization and life skills

• Building confidence and independence

• Caregiver collaboration and support

Coach Joy brings the same strengths-based, faith-informed philosophy that has guided PathFinders Family Coaching while providing services within the scope of each individual's approved waiver program and authorized service plan.

How to Get Started

1. Contact your Service Coordinator or Case Manager to determine whether your waiver program includes habilitation services through the Consumer Directed Services (CDS) option.

2. Ask whether you have approved CDS habilitation hours available within your service plan.

3. Contact PathFinders Family Coaching to discuss availability and whether our services are a good fit for your family's goals.

4. If appropriate, work with your Financial Management Services Agency (FMSA) to complete the required employment and onboarding process.
